Nathana leads At Home Visit, PLLC with a personal approach to primary care. Based in Grayson and serving the surrounding Eastern Kentucky community, the practice brings care closer to the people who need it.

How do I become a patient?
Call (606) 315-3700. Our office will talk with you about your needs, confirm your location and visit options, and explain the next steps for getting started.
Where do you offer home visits?
Our published service area includes Carter, Boyd, Greenup, Elliott, Rowan, Lewis, Lawrence, and Morgan counties, along with Paintsville and South Shore, Kentucky. Call to confirm availability at your address.
Do you accept my insurance?
Please call the office with your insurance information so we can confirm current participation, benefits, and any costs before your appointment. Coverage and eligibility can vary by plan and service.
What should I have ready for my first visit?
When scheduling, ask our office what to prepare. Having your insurance card, a current medication list, and contact information for your other healthcare providers available can help us get to know you.
Do you offer weight-management care?
Contact our office to discuss current weight-management services, visit options, and eligibility. Our team can explain what is available and help you decide on the next step.
Is there a patient portal?
Yes. Use the Patient portal link to access the practice’s Athenahealth portal. Contact the office if you need help getting access.
Can I request a same-day appointment?
Same-day or next-day visits may be available. Call our office to discuss your needs and current availability. If you are experiencing a medical emergency, call 911.
